What is the payment procedure?
Consults are payable on the day.
Prior to your appointment we will need:
A referral from your GP. You will also need to notify us if you have had any tests done – Blood Tests, CT scan’s, X Rays, MRI’s or any other test that is relevant to the visit with Dr Perry.
For you first consult we ask that you bring with you:
- Medicare Card
- Private Health Information (if applicable)
- Glasses if you need them to fill in forms
